Health Insurance

Plan A – $300 Deductible

  • Pays 80/20 at Network Providers
  • Pharmacy Card
  • Includes short term disability for hourly employees

Plan B – $500 Deductible

  • Pays 60/40 at Network Providers
  • Pharmacy Card

Plan C – $250 Deductible

  • No Network restrictions
  • Prescriptions reimbursed after deductible is met

Basic Life

  • 100% Employer Paid

AD&D

  • 50% Employer Paid
    • Voluntary Life

      • Premiums based on employee’s age and amount requested; spouse and children can be covered.

      Long Term Disability (For salaried employees)

      • Class 1 – 66.7% Pre-disability earnings
      • Class 2 – 60% Pre-disabilaty earnings

      Vision

      • Single and Family Coverage – $10 copay
